Steps to download ASSESSMENT FORM

1. Right click on ASSESSMENT FORM
2. Select “Save Link As“
3. Select where to save the file (My Document or Desktop)  and Hit Save
4. Fill form and upload in the Assessment Form field in Step 4

Steps to download ACKNOWLEDGEMENT/EXAMINATION SLIP

1. Right click on ACKNOWLEDGEMENT/EXAMINATION SLIP
2. Select “Save Link As“
3. Select where to save file (My Document or Desktop) and Hit Save
4. Click on picture box to upload Picture
5. Fill form and click on print botton when done

(Bring this slip along with you for the Entrance Examination)
